Booking Forms & Information
Booking forms for the next Holiday Club can be found below. Please make sure these are returned to Unique Kidz and Co by the deadline date on the front page. We can also issue forms in the post. Please call the office to organise this.
If your child has never attended Unique Kidz and Co or 4Ever Unique before, please contact the office before completing a booking form. Our staff will be able to advise you on our services and invite you for a visit.

In October Holiday Club, anyone over the age of 16 can book to attend 4Ever Nightz from 4pm-6pm. If you are interested in this service please contact Anne-Marie in the office on 01524 831132.
